What You'll Do:

โ€ข Provide daily engagement and support to adults with IDD in program activities and community integration.
โ€ข Assist with personal care needs (e.g., toileting, feeding, hygiene, grooming) depending on individual support plans.
โ€ข Foster social skills, independence, and meaningful community participation through structured activities.
โ€ข Lead and facilitate group activities and individualized skill-building experiences that align with each participantโ€™s interests and goals.
โ€ข Support vocational exploration, community outings, hobbies, or leisure activities as part of a meaningful day agenda.
โ€ข Help participants pursue employment-related activities (e.g., pre-employment training, job coaching) when part of the program model.
โ€ข Maintain accurate documentation including progress notes, incident reports, and daily logs for each participant.
โ€ข Report observations on behavior, participation, goals, progress, and any changes in needs.
โ€ข Contribute to periodic reviews and updates of individual person-centered plans (PCP) as needed.

โ€ข Provide or oversee safe transport to off-site activities, appointments, and community outings when required by the program.
โ€ข Ensure participant safety in community settings, monitor behavior, and follow risk management practices.
โ€ข Work collaboratively with other staff, supervisors, families, and interdisciplinary teams.
โ€ข Promote participant self-determination, choice, and dignity, using person-centered practices.
โ€ข Participate in staff meetings, training, and professional development to improve service delivery.

What We Require:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.   • Must Possess a valid driverโ€™s license with no more than 3 points and have access to a vehicle for work purposes.
  • Must have proof of car insurance.
  • High School Diploma (or equivalent) and a minimum of two (2) yearsโ€™ experience working with adults diagnosed with varying intellectual disabilities and/or developmental disabilities.
  • Must be able to work individually as well as within a team. Must be able to multi-task.
  • Must be able to work with โ€œhigh-riskโ€ consumers who have development or intellectual disabilities as well as mental illnesses as well and physical disabilities.

Qualifications:

  • Compassionate and caring nature
  • Excellent communication and interpersonal skills
  • Training in CPR/First Aid, Medication administration training if applicable
  • DDA core trainings.
  • Ability to work independently and as part of a team
  • Patience, creativity, and ability to adapt activities to individual needs
  • Experience working with individuals with disabilities (preferred).
  • Ability to pass background checks and FBI fingerprinting.
