Job description

Part Time/On Call LPN Health Coordinator
LPN needed to work with seniors and adults with disabilities in an Adult Day Program located in Wake County, NC.
The Primary Functions of this position are:
  • Perform skilled nursing tasks;
  • Monitor medical needs of participants;
  • Maintain accurate documentation for Total Life Center participants;
  • Provide training and supervision to Total Life Center and Resources for Seniors, Inc. staff.
The qualified candidate will have proven successful experience in:
  • Conducting health assessments;
  • Knowledge of the physical and emotional aspects of aging;
  • One year experience in the provision of activities and services, preferably with impaired adults;
  • One year experience with personal care and creating care plans;
  • Assisting participants with daily living activities (toileting, feeding, transferring, and bathing, etc.).
Additional qualifications required:
  • Valid Licensed Practical Nurse license through the North Carolina Board of Nursing;
  • Certification in CPR and First Aid training.
The ideal candidate should be flexible, patient, and available to work in a team environment with minimal supervision varied hours on Monday-Friday, no nights or weekends. The Health Coordinator is classified as a part time, non-exempt position.
