Job description

Case Manager
Position Summary:
Life'sWork of Western PA is seeking a compassionate, organized, and resourceful Case Manager to provide case management services to children, youth, and families. This position partners closely with referral sources, families, community providers, and county agencies to support positive outcomes, strengthen families, and connect clients with critical community resources.
The Case Manager conducts regular in-home visits, develops service plans, coordinates care, advocates for clients, completes assessments, and maintains accurate documentation in compliance with agency and county requirements. On average, Case Managers serve approximately 2-3 clients per day and have the flexibility to work a non-traditional schedule that meets client needs, including evenings and weekends as necessary.
Essential Job Responsibilities:
Client Services & Case Management
• Provide in-home case management services based on referral needs and treatment goals.
• Partner with the Westmoreland County Children's Bureau (WCCB), families, and community providers to identify and achieve client-centered goals.
• Conduct regular in-person visits with clients and families.
• Assess client needs and develop strategies that promote safety, stability, and independence.
• Connect clients with community resources, including:
• Housing assistance
• Mental health services
• Drug and alcohol treatment
• Employment and educational supports
• Other community-based resources
• Advocate for clients and assist in navigating complex systems and services.
• Provide crisis intervention and support when needed.
• Transport clients to appointments and services as authorized.
Documentation & Compliance
• Complete assessments, releases, background information, and all required intake documentation.
• Create and maintain client files in accordance with WCCB and agency standards.
• Develop and update service plans, weekly reports, contact summaries, and other required documentation.
• Maintain accurate, concise, and timely records within the Horizon system.
• Meet all county, agency, and contractual documentation deadlines.
• Complete North Carolina Family Assessments for applicable cases.
• Prepare and submit monthly reports to CYF Case Workers and Supervisors.
• Completes all required trainings and attends mandatory meetings as scheduled.
Collaboration
• Maintain regular communication with CYF Case Workers, Navigators, community partners, and service providers.
• Participate in case planning and service coordination meetings.
• Research and leverage available community resources to address client needs.
• Work collaboratively with internal and external stakeholders to support positive outcomes for families.
Qualifications:
• Education: Bachelor's degree in social work or related field; Master's degree preferred; Associate degree will be considered with 3-5 years of equivalent professional experience
• Must possess basic computer skills and ability to navigate Microsoft Office tools.
• Possess a valid Pennsylvania Driver's License, access to an automobile and current automobile insurance.
• Act 33/34, Criminal Background, and FBI Clearances are required.
• Flexibility with regard to a work schedule to meet the availability of the clients. Must be available for nontraditional hours (evenings, weekends, etc.).
• A willingness to transport clients is also required.
• Familiar with the human service field, the juvenile court system, child development, conflict resolution, and experience with individual and family counseling
• Must maintain client, program and agency information in a confidential manner and discuss such information privately with only appropriate personnel.
• Expected to treat all clients with respect and professionalism.
• Effective verbal and written communication skills.
• Must be able to use creative problem solving and thinking outside the box to meet needs of clients.

Compensation
• Bachelor's Degree: $45,000 - $48,000 annually Based on experience
• Master's Degree: $50,000 - $55,000 annually Based on experience
• Associate Degree: $35,000 - $40,000 annually Based on experience
• Performance Bonus: Case Managers who exceed the monthly productivity standard of 150 billable hours are eligible for a monthly bonus program.
