Description/Summary
The full-time Case Aide provides, upon request and under the direction of the Supervisor and Case Workers, direct services to children and families involved with the CYF Unit. Duties include but are not limited to supervised visitation, transportation, home visits, parent education and support, school liaison work, providing support to CYF caseworkers, and other tasks as directed by the CYF supervisor. This position also requires accurate record keeping and case documentation, filing and other errands and paperwork as assigned by CYF supervisor or caseworkers.
Essential Duties and Responsibilities
ESSENTIAL DUTIES AND RESPONSIBILITIES
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required or assigned.
? Provide supervised visits and oversight.
? Provide direct services to clients at the request and direction of the supervisor and care managers, including phone calls, face-to-face observation, gathering signatures from client/family, making collateral calls and making referrals.
? Coordinate and/or provide transportation to clients if other transportation is not available.
? Provide parent education as assigned and appropriate.
? Act as liaison between agency and school systems.
? Perform home visits as assigned.
? Provide observation, listening and communication skills to assess and provide for continuity of care.
? Assist and support Supervisor and Case Workers as assigned.
? Perform miscellaneous duties as time permits.
? Maintain client/agency records in accordance with federal, state and agency policies.
? Maintain the confidentiality of client information and protected health information as required by federal & state regulations and comply with accepted professional standards.
? Attend staff training /development as assigned and appropriate.
? Participate in unit meetings, client staffings and other meetings as assigned and appropriate.
Qualifications
MINIMUM TRAINING AND EXPERIENCE REQUIRED TO PERFORM ESSENTIAL JOB FUNCTIONS
? Associate's degree in Human Services or closely related field.
? One year of related direct service experience working with children and families. An equivalent combination of education and clearly documented training or experience that provides the required knowledge, skills and abilities may be considered.
? Working knowledge of computers, computer programs, typing, and data entry.
? Current WI driver's license and unlimited access to reliable, insured transportation.
