Job Description
  • Day Shift LPN Needed as Medical Abstractor.
  • Reviews post-discharge medical records for specific quality of care indicators as defined by various internal and external agencies.
  • Abstracts findings into designated database and maintains accurate list of variations from expected indices.
  • QC's other abstractors Missed Opportunities.
Qualifications
  • Minimum of two years practical clinical and/or auditing experience is required.
  • Active LPN
  • Knowledge and exercise of CMS and Joint Commission Core Measures and Stroke GWTG.
  • A basic knowledge of DRG's and coding is essential. Proficiency in abstracting of CMS and TJC Core Measures for the HIPQRP (Hospital In Patient Quality Reporting Program), HOPQRP (Hospital Out Patient Quality reporting Program) and IPFQR( In Patient Psychiatric Facility Quality Reporting)is required.
  • Familiarity with Premier QMR (Quality Measure Reporter) Database and Cerner eMR (electronic Medical Record) is required. Ability to learn other databases as required.
