Position Summary:  

 

The Respiratory Therapist COPD Transitional Navigator provides comprehensive education to patients diagnosed with chronic obstructive pulmonary disease (COPD). 

 

Essential Functions and Responsibilities Includes the Following:

 

  1. Understands and adheres to the WPH Performance Standards, Policies and Behaviors Utilizes written education materials along with educational aides such as handouts, lung models, and videos as deemed appropriate. When applicable, will provide referrals for pulmonary rehab and pulmonary function testing. Acts as a liaison to physicians, case managers, and discharge planners to ensure proper length of stay, adequate education, and safe discharge.  Provides follow-up care via phone to answer questions and ensure compliance and appointments with PCP. 
  2. Education will include: 
    • Normal lung anatomy and physiology
    • Physiologic changes caused by COPD
    • Medication classifications
    • Patients specific medication – what, when, how, and why to take. (Action Plan) 
    • Recognizing exacerbations vs. “just a bad day” – (Action Plan) 
    • Pulmonary rehab and exercise
    • Smoking cessation
    • Oxygen use
    • Other home equipment use
    • Nutrition specific to COPD patients
  3. Performs all aspects of Respiratory Therapy to both in and out patients. Categories will include adult, adolescent, infant, neonatal, pediatric and geriatric patients.
  4. Performs all other related duties as assigned 

Education & Experience Requirements

  • Must be a graduate of an AMA approved program for Respiratory Therapy. 
  • Successful completion of department/hospital Orientation Program. 
  • Must hold a current CPR/BLS certification. 
  • ACLS certification required
  • Current and valid NYS Respiratory Care License
  • Credentialed as a Registered Respiratory Therapist (RRT) 0r will be within 6 months of accepting position. 
  • Expert knowledge of COPD, clinical respiratory assessment, and pulmonary diseases. 

 Preferred: 

  • Credentialed as a COPD educator via the American Association of Respiratory Care (AARC) 
  • Certified as a Tobacco Cessation Specialist
  • Core Competencies

  • Ability to juggle multiple priorities at the same time
  • Ability to use good judgment and has sound decision making skills
  • Team player and willing to go above and beyond to get the job done ·    Maintains confidentiality and uses discretion at all times

Physical/Mental Demands/Requirements & Work Environment

May be exposed to chemicals necessary to perform required tasks. Any hazardous chemicals the employee may be exposed to are listed in the hospital’s SDS (Safety Data Sheet) data base and may be accessed through the hospital’s Intranet site (Employee Tools/SDS Access). A copy of the SDS data base can also be found at the hospital switchboard, saved on a disc.
