Job description

S:US is seeking a Case Manager for their homeless prevention program. 

This position is split between two program locations:

Location: West 181st Street (St. Nicholas Avenue) and Rikers Island, New York, NY. Employees are required to be onsite at  Riker's Island location at least 2 days a week.

This is a multi-site position that requires regular travel between both locations.

SCOPE OF ROLE:

The Case Manager provides comprehensive case management services to individuals and families at risk of homelessness. This role supports participants across both the West 181st Street office and the Rikers Island program location, ensuring continuity of care, access to housing resources, and successful service coordination to promote long-term housing stability and self-sufficiency.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provide case management services to individuals and families at risk of homelessness.
  • Conduct assessments and develop individualized service plans to address housing barriers.
  • Connect participants to housing resources, benefits, and supportive services.
  • Assist clients with financial assistance applications and benefit enrollment or reinstatement.
  • Advocate for clients and coordinate services with community providers and landlords.
  • Monitor participant progress and maintain accurate case records and documentation.
  • Support clients in achieving housing stability, self-sufficiency, and long-term success.
  • Collaborate with internal and external partners to ensure participants receive comprehensive services.
  • Other duties as assigned.

Qualifications

REQUIREMENTS:

  • HS Diploma, GED, or AA degree in related field and two years qualifying experience

PREFERRED QUALIFICATIONS & SKILLS

  • MSW or bachelor's degree preferred in related field.
  • Knowledge of the Department of Homeless Services, the Human Resources Administration, and entitlements.
  • Knowledge of the complex interaction of multiple factors of homelessness, incarceration, and substance abuse, mental health and domestic violence is preferred with sensitivity to cultural and gender specific issues with both individuals and families.
