Key Responsibilities
  • Identify individuals and communities facing challenges, specifically those related to end-of-life care and other significant life transitions.
  • Conduct comprehensive psychosocial assessments to understand the unique needs, strengths, and support networks of your clients.
  • Implement personalized treatment plans, including individual, family, or group therapeutic sessions, leveraging evidence-based practices.
  • Coordinate care efforts involving team consultations and facilitate case management to ensure optimal client outcomes.
  • Serve as a liaison with community agencies, helping clients access important resources such as financial aid, healthcare support, and mental health services.
  • Immediately respond to crisis situations, such as instances of child abuse or acute mental health emergencies, providing necessary intervention and support.
  • Regularly monitor client progress and follow up to guarantee continued improvement and adaptation to their changing circumstances.
Patient Population / Client Focus

This role centers on serving a diverse patient population within the hospice context, offering support to individuals and families affected by terminal illnesses and significant life changes. Qualifications
  • A Masterโ€™s Degree in Social Work (MSW) from an accredited institution.
  • A valid and active Licensed Clinical Social Worker (LCSW) license in California is required.
  • A minimum of 2 years of experience working as a licensed social worker in a clinical setting.
Preferred Experience
  • Experience in hospice, palliative, or end-of-life care is highly preferred.
