PRIDE Health is seeking a local contract Physical Therapist for a local contract job in Jackson County, Mississippi.

Physical Therapist – Home Health

Position Summary

We are seeking a Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout Jackson County, MS. The PT will evaluate patients, provide treatments, develop plans of care, and coordinate with the healthcare team to support positive patient outcomes
Location: Jackson County, MS
Schedule: Monday–Friday
Hours: 40 hours/week
Contract: 16 weeks
Pay: $55–$60/hr on W2
Weekend: Rotating weekends; schedule adjusted to maintain 40 hours
Mileage: Reimbursed between patient visits
Dress Code: Royal Blue

Responsibilities
  • Perform patient assessments, evaluations, and treatments.
  • Develop and update individualized plans of care.
  • Complete OASIS assessments and documentation.
  • Supervise PTAs as required.
  • Maintain timely and accurate clinical documentation using HCHB.
  • Complete documentation by 8:00 AM the following morning.
  • Collaborate with patients, families, physicians, and the care team.
Requirements
  • Active Mississippi PT license.
  • Home health experience required.
  • OASIS and HCHB experience required.
  • Strong communication, teamwork, flexibility, and professionalism.
  • Ability to work independently in patients' homes.

Travel: This is a home health position requiring travel between patient homes. Mileage is reimbursed between visits.
