MobileMedix Plus is seeking experienced, compassionate Contract (1099) Paramedics to serve as Community Paramedics, providing in-home care and support for patients referred by hospitals, clinics, and physician offices.


This is a unique opportunity to practice at the top of your license while building meaningful, ongoing relationships with patients and addressing the root causes of poor health outcomes.


About the Role


As a Community Paramedic, you will conduct scheduled in-home visitsโ€”typically once per week for up to four weeksโ€”to support patients with chronic disease management and care coordination.


Youโ€™ll play a critical role in reducing unnecessary 911 calls, emergency department visits, and hospital readmissions by proactively addressing medical and social needs in the home setting.


This is a self-directed, field-based position where you will work independently using your personal vehicle, while staying connected with a collaborative care team.


Key Responsibilities

  • Conduct in-home patient visits and ongoing wellness checks
  • Monitor vital signs and assess overall health status
  • Perform medication reconciliation and adherence support
  • Provide chronic disease and health education
  • Complete home safety and fall risk assessments
  • Identify and address social determinants of health (SDOH) barriers
  • Coordinate care and communicate updates with providers and care teams
  • Assist with scheduling follow-up appointments (PCP/specialists)
  • Facilitate access to durable medical equipment (DME) and community resources
  • Document all patient interactions accurately and timely

Schedule & Compensation

  • Typical hours: Monday โ€“ Friday, 9:00 AM โ€“ 5:00 PM
  • Flexible scheduling based on patient needs
  • Starting pay: $29.00/hour (minimum)
  • 1099 Contract position
  • Opportunity for transition to full-time employment with benefits

Qualifications

  • Active Paramedic certification (State of Florida or eligible)
  • Minimum 3 years of full-time paramedic experience (911, ED, hospital, public health, or similar)
  • Strong clinical assessment and decision-making skills

  • Ability to work independently with minimal supervision
  • Reliable personal vehicle and valid driverโ€™s license

Preferred:
