Job description

The community social worker along with the community case managers identifies patient's and family's psychosocial and environmental needs related to health care. The community social worker connects patients and their families to the appropriate community resources to successfully maintain or improve their desired level of care. The community social worker will meet with patients and families in their homes to assist with problem solving, application paperwork and determining needed resources.
Minimum Qualifications
  • Bachelor's Degree in Social Work, Psychology or Human Services field or equivalent work experience.
  • At least two years' social work experience

Preferred Qualifications
  • Master's Degree in Social Work, Psychology or Human Services field
  • Certification in Case Management
  • One-year acute care experience

Skills
  • Excellent communication skills with the ability to interact with a diverse population.
  • Strong organizational skills and able to work independently.
  • Able to work in a fast-paced environment and multi-task.
  • Understands when to ask for assistance from Community Case Manager or Director of Case Management.
  • Knowledge of long-term care resources and qualifying requirements; Medicare, Medicaid, third party payors; financial, transportation, pharmaceutical assistance and other community and health systems resources for assisting with patient needs.
