Take the lead in making a meaningful difference in the lives of residents and their families. A skilled nursing facility in the Hazlet area is looking for a Director of Social Work to oversee social services and play a key role in resident support and care coordination.

The Role
  • Direct and oversee the facility’s social services program

  • Manage discharge planning and coordinate successful transitions

  • Advocate for residents and serve as a resource for families

  • Work alongside nursing, therapy, and other members of the interdisciplinary team

  • Address psychosocial needs and help resolve resident concerns

  • Maintain compliance with applicable state and federal requirements

Qualifications
