Job description

Job Title: Community Resource Navigator
Reports To: Executive Director & Director of Operations
FLSA Status: Non-Exempt
Salary: $50,000 annually + benefits
Schedule: Full-time, 37.5 hours/week (typically Monday–Friday, 9:00 AM–4:30 PM). Occasional evenings and weekends for community outreach and center special events.

Are you passionate about helping individuals and families access the resources they need to thrive? Join our team as a Community Resource Navigator and play a vital role in connecting community members to critical services like housing, transportation, and social supports.
Position Summary:
The Community Resource Navigator will provide screenings, navigation, direct services, and enhanced care management to individuals and families in need of support in housing and transportation.
What You’ll Do:
As a Community Resource Navigator, you will:
  • Provide comprehensive case management, including intake assessments, service planning, and ongoing support.
  • Conduct and review Life Needs Assessments and ensure follow-up on identified needs.
  • Develop and implement individualized care plans in collaboration with staff and partners.
  • Connect individuals and families to housing, transportation, and other essential services.
  • Maintain consistent communication and support with assigned participants.
  • Utilize platforms such as Salesforce and Unite Us to track services and referrals.
  • Represent the organization at community events, outreach efforts, and partner meetings.
  • Support initiatives connected to the Healthy Alliance Social Care Network.
  • Collaborate across departments to strengthen programs and community impact.
  • Participate in professional development opportunities and ongoing learning.
  • Contribute to a positive, mission-driven team culture.
  • Take on additional responsibilities as needed to support organizational goals.

  • Ability to work independently, prioritize tasks, and manage time effectively.

  • Reliability and punctuality, both in the office and at off-site meetings.
Qualifications:
  • Associate’s degree or higher (or equivalent combination of education and experience).
  • Experience in case management, social services, or community-based work preferred.
  • Familiarity with databases or case management systems (Salesforce, Unite Us a plus).
  • Valid driver’s license and access to a reliable vehicle for daily use.
  • Ability to pass a background check.
Technical Skills:
  • Proficiency in Microsoft Office (Word, Excel, Outlook).
  • Ability to learn and use database systems and other relevant software.
