The required shift for this position is: Monday- Friday 8:00AM- 4:30PM
JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.
Prototypes' Outpatient Behavioral Health Centers provide comprehensive mental health services for men, women and children. Services range from prevention and early intervention to intensive services for those with more serious or chronic conditions. Staff provide services at Prototypes and in the community. These can include individual, group and family therapy, psychiatric treatment, medication management and medication-assisted therapies, case management, and referrals and coordination with primary care clinics.
The Care Coordinator II is an integral part of an interdisciplinary team that supports our clients through assessment of their needs and strengths in support of their health and recovery. Care Coordinator II will perform counseling in individual and group settings, deliver curricula in accordance with program specific requirements and case management, and assist clients in navigating systems of care while providing a supportive treatment environment.
KEY RESPONSIBILITIES
Treatment Setting

  • Actively participate in scheduling and initial assessments/intakes, embodying the welcoming and engagement philosophy.
  • Maintained an assigned caseload of clients.
  • Provide individual and group counseling.
  • Facilitate various group sessions (e.g., process groups, psychoeducational groups, relapse prevention, 12 steps).
  • Work with clients to develop individualized service/treatment plans with measurable goals and objectives.
  • Conduct regular follow-up to ensure clients' adherence to the treatment plan.
  • Perform crisis intervention and communicate with the treatment team as unforeseen situations arise.
  • Provide case management services related to clients' psychosocial needs (housing, employment, legal issues, health issues).
  • Assist in planning and implementing drug-free recreational activities and events for clients and their families.
  • Assist in community outreach, referrals (warm hand-offs), and client follow-ups.
Documentation
  • Maintain confidential client records and prepare client progress reports and other required documentation in a timely manner.
  • Accurately enter data in various electronic systems for all clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
Administration and Other Duties
  • Establish community relations and linkages with other service providers and community services.
  • Actively participate in clinical supervision, case conferences, and treatment team reviews of client progress.
  • Attend meetings and actively participate.
  • Complete assigned training on time.
  • Meet service delivery goals.
  • Read and respond to communications in a timely manner.
  • Maintain full compliance with credential requirements.
  • Assist with the onboarding of new hires and training or mentoring of new and existing employees, interns, or ambassadors.
And perform other assigned duties.
QUALIFICATIONS
Education, Certification, or Licensure
  • High school diploma or equivalent required. Bachelor's degree in Health and Human Services, Social Work, Psychology, or other related field preferred.
  • Registration as a Substance Use Disorder Counselor from an accredited California agency (CCAPP, CAADE, CADTP).Valid First Aid and CPR certification or ability to obtain within 30 days of hire.Valid California Driver's License and access to registered and insured transportation.
Experience
  • At least one year of experience as a registered substance abuse or behavioral disorder counselor or recovery specialist in community mental health setting.
  • Experience carrying a caseload or has the ability to carry a caseload of clients.
