About the Role:

The Community Support Provider plays a vital role in enhancing the quality of life for individuals within the community by offering personalized assistance and support services. This position focuses on fostering independence, promoting social inclusion, and ensuring access to necessary resources for community members. The role requires building trusting relationships with clients, understanding their unique needs, and collaborating with multidisciplinary teams to develop and implement effective support plans. The Community Support Provider acts as an advocate, helping individuals navigate social services, healthcare, and other community resources. Ultimately, this position contributes to creating a supportive and empowering environment that enables individuals to thrive within their communities.

Minimum Qualifications:

  • High school diploma or equivalent.
  • Experience working in community support, social services, or a related field.
  • Valid driverโ€™s license.
  • Ability to pass background checks and comply with state and federal regulations.

Preferred Qualifications:

  • Experience working with diverse populations, including individuals with disabilities or mental health challenges.
  • Knowledge of local community resources and social service systems.
  • Bilingual English/Spanish.

Responsibilities:
