Job description

A home care agency in Raleigh is looking for a Referral Liaison to promote the Agency's services and develop referral relationships within the community.

The Referral Liaison shall:
  • Establish and maintain relationships with hospitals, physician offices, rehabilitation facilities, case managers, social workers, senior centers, and other referral sources.
  • Educate referral sources regarding Agency services.
  • Conduct marketing visits and community outreach activities.
  • Track referral activity and maintain records of contacts and outcomes.
  • Submit weekly and monthly activity reports.
  • Represent the Agency professionally and ethically at all times.

Compensation is negotiable.
