Work Environment

As a remote position, the LCSW will have the flexibility to manage their schedule while primarily functioning from home. Team members engage through virtual meetings, collaborative case discussions, and community outreach initiatives. While the standard work hours are Monday to Friday from 8:00 AM to 4:30 PM, some flexibility will be required to accommodate client needs, including occasional evening or weekend hours. On-call availability is also a vital component of the role, ensuring urgent client needs are addressed promptly.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments of seniors, considering their emotional, mental, and social needs.
  • Develop tailored care plans in collaboration with clients and their families, focusing on their individual goals, preferences, and challenges.
  • Provide counseling and emotional support, helping clients navigate significant life transitions, such as relocation to assisted living or dealing with bereavement.
  • Advocate for clientsโ€™ rights, ensuring they have access to necessary community resources and services.
  • Collaborate with an interdisciplinary team, including healthcare providers and caregivers, to enhance each client's care experience.
  • Perform regular home visits to monitor clients' well-being, assess living conditions, and address safety concerns.
  • Educate clients and their families about available support groups and community programs that foster social engagement.
  • Maintain detailed and accurate documentation of client interactions and care plans in an electronic health record (EHR) system.
  • Stay informed about best practices and policies in social work to ensure compliance with ethical standards.
Patient Population / Client Focus

The primary focus of this role will be on seniors in the Palmer community facing various psychosocial challenges. The LCSW will directly engage with elderly clients and their families to provide support and facilitate access to essential services. Qualifications
  • Master's degree in Social Work from an accredited institution.
