Responsibilities:

  • Assesses and evaluates the health care needs of patients and families with consideration regarding physiological, psychological, social and environmental factors. 
  • Develops a care plan based on patient needs, physician orders, nursing assessment parameters and identified goals that are respectful of patient, family, community and company resources.โ€ฏ  
  • Implements the care plan and revises it whenever necessary by regularly assessing, observing and evaluating the patientโ€™s condition, needs and response to care, and makes appropriate nursing judgments and decisions for care plan revision.โ€ฏ 
  • Initiates and applies appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques.โ€ฏ  -
  • Administers medications and treatments as prescribed by the physician and performs nursing procedures and techniques.โ€ฏ 
  • Delegates responsibility appropriately and supervises ancillary personnel in a manner that will assure quality care and compliance with the care plan.โ€ฏ 
  • Completes, maintains and submits all required documentation that is timely, accurate and relevant.   

Qualifications:

  • Current NYS Registered Nurse License 
  • At least one year of hospital or nursing home experience (required)
  • 1 year of community health experience in a Certified Home Health Agency (CHHA)
