Job description

CenterLight Healthcare PACE Nurse

Bring exceptional nursing care directly to the community.

At CenterLight Healthcare PACE, you'll build lasting relationships with older adults while helping them remain safe, healthy, and independent in their own homes. Unlike traditional home care, you'll be part of an Interdisciplinary Team (IDT) where your clinical expertise directly shapes each participant's individualized care plan.

What You'll Do

  • Provide skilled nursing care in participants' homes and across the continuum of care.
  • Complete comprehensive nursing assessments and UAS-NY evaluations.
  • Develop and update individualized care plans with the IDT.
  • Coordinate care with physicians, hospitals, specialists, nursing facilities and community providers.
  • Administer medications and skilled nursing treatments.
  • Educate participants and caregivers on disease management, medications and safety.
  • Identify changes in condition and intervene early to prevent avoidable hospitalizations.
  • Complete timely, accurate clinical documentation.

What We're Looking For

  • Current New York State Registered Nurse (RN) license
  • Graduate of an accredited nursing program
  • Minimum one year of RN experience in Home Health, Acute Care, Medical-Surgical and/or Critical Care
  • Valid driver's license and reliable vehicle
  • Computer proficiency
  • Strong communication and organizational skills
  • BSN
  • Experience with frail or elderly populations
  • UAS-NY assessment experience
  • PACE, Managed Care, Home Care or Case Management experience
