Midland Care Connection

Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. ย Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.

Community Home Health Aideย 

Helps older adults remain safely and comfortably in their own homes. The job focuses on assisting clients with daily personal care needs such as bathing, dressing, grooming, using the restroom, eating, and moving around the home. Home health aides also provide companionship and emotional support, which is especially important for seniors who may live alone or have health challenges.

In addition to personal care, aides often help with light housekeeping, laundry, meal preparation, and medication reminders. They follow a care plan created by nurses and other healthcare professionals and report any changes they notice in a clientโ€™s condition.ย The position is hands-on and physically active. Aides may help clients transfer from beds or chairs, assist with walking, and travel between clientsโ€™ homes during the workday.ย 

Pay Rate
$20.50+/hour

Shift Differentials
$1.00 Weekdays | 5:00pm โ€“ 8:00pm
$1.50 Weekends | 8:00am โ€“ 5:00pm
$2.00 Weekends | 5:00pm โ€“ 8:00am

Required

  • Current Home Health Aide (HHA) certification in the state of Kansas
  • High school diploma or GED
  • Ability to provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility
  • Basic knowledge of infection control, safety practices, and patient rights
  • Ability to observe, document, and report changes in a clientโ€™s condition to nursing staff or supervisors
  • Compassionate, respectful, and patient-centered approach to care
  • Strong communication and interpersonal skills
  • Ability to work independently in clientsโ€™ homes while following care plans
  • Physical ability to perform job duties, including lifting, bending, and standing for extended periods
  • Valid driverโ€™s license, reliable transportation, and willingness to travel to clientsโ€™ homes
  • Ability to pass background checks, health screenings, and required trainings

Preferred

  • Previous experience working with older adults, individuals with chronic illness, or those with mobility or cognitive impairments
  • Prior home health, hospice, palliative care, or PACE program experience
  • Experience assisting with light housekeeping, meal preparation, and medication reminders
  • Familiarity with interdisciplinary care teams and community-based healthcare models
  • CPR/First Aid certification
  • Demonstrated ability to provide emotional support and companionship to clients
