Job description

Midland Care Connection

Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.

Visting Nurse Association

Specializes in home hospice that provides compassionate, end-of-life care to patients in the comfort of their own homes. Their skilled nurses focus on managing pain and symptoms, supporting emotional and spiritual needs, and guiding both patients and families through the hospice journey. Working closely with an interdisciplinary team, the VNA ensures dignity, comfort, and quality of life during a patient’s final days while offering education, resources, and respite support to caregivers.

Certified Nursing Assistant 

Provides hands-on personal care and comfort support to patients receiving end-of-life care at home. Working under the supervision of a registered nurse, the aide assists with bathing, grooming, dressing, toileting, turning, and repositioning to maintain comfort and prevent skin issues. They may also help with meals, feeding, light housekeeping, laundry, and changing bed linens. In addition, aides observe the patient’s condition and report any changes to the hospice nurse. Their role focuses on maintaining dignity, comfort, and quality of life while also giving family caregivers practical support and relief.

Hours
Monday - Friday | 8:00am - 5:00pm

Pay Rate
$18.15+/hour

Bonus
***$1,000***
To be paid in two increments at 6 months and 1 year of employment
+ Paid training to obtain HHA license after 60 days

Required

  • Current Certified Nurse Aide certification in Missouri & Kansas
  • Willingness to enroll in HHA certification course within first 60 days of employment
  • Ability to provide companionship and assist with daily living activities (e.g., mobility, meal support, safety) in clients’ home
  • Help clients stay safe and comfortable in their own homes
  • Travel within the community to deliver care and support

Preferred

  • At least one year of experience working as a CNA in hospice, home health, long-term care, or assisted living setting
  • CPR and First Aid certification
  • Strong communication and interpersonal skills
