Job Summary and Responsibilities

As our LVN Treatment Nurse, you will be a cornerstone of our patient care team, providing specialized skin care and clinical support to residents under the guidance of Registered Nurses and the Nurse Manager.

Every day, you will monitor patient status, execute individualized treatment plans, manage medications, and coordinate with the interdisciplinary team to ensure that our residents receive high-quality, compassionate care that meets all CMS and OBRA standards.

To be successful in this role, you will bring a sharp eye for clinical detail, a commitment to patient advocacy, strong communication skills, and a dedication to upholding the mission and core values of Community Hospital of San Bernardino.

Key Responsibilities:

  • Execute primary skin care and advanced wound treatment therapies in strict adherence to CMS, OBRA, and hospital-established medical protocols.
  • Conduct regular skin assessments, including staging and measuring pressure ulcers, while promptly reporting changes in condition to the clinical team.
  • Administer medications and treatments as ordered by physicians, ensuring accurate documentation and proper management of narcotics and supplies.
  • Collaborate with the Wound Care Committee and RN charge nurses to develop, implement, and evaluate comprehensive resident care plans.
  • Supervise and audit the documentation and daily tasks of nurse aides to ensure consistent, high-quality patient care across the unit.
  • Maintain compliance with all hospital policies, including infection control, patient privacy, and active participation in quality improvement and continuing education activities.
Job Requirements

Required

  • One (1) year experience in Long Term Care and Subacute
  • Graduate of an accredited Licensed Vocational program
  • Licensed Vocational Nurse: CA, upon hire 
  • Basic Life Support - CPR, upon hire 
  • Expanded IV Therapy Course, within 90 Days
