About the role

If you're a nurse looking for a way out of the bedside grind without giving up patient care, read on.

As an RPM Nurse at CGM, you manage a panel of patients living with chronic conditions like hypertension, diabetes, CHF, and COPD. You review their daily vitals, catch the trends that matter, run monthly check-in calls, and loop in providers when something needs attention. Real nursing, from your home office.

What you'll do
  • Review daily vitals (BP, weight, glucose, SpO2) from your panel and act on what you find.
  • Triage out-of-range readings and escalate to the supervising provider with a clear recommendation.
  • Document every review, intervention, and call to meet CMS billing requirements.
  • Run monthly check-in calls: assess symptoms, review meds, reinforce the care plan, answer questions.
  • Onboard new patients and walk them through their devices.
  • Bridge the patient and the provider's office. Relay concerns, follow up on orders, close the loop.
  • Flag access barriers (transportation, cost, caregiver support) and connect patients to resources.
What we're looking for
  • Active, unrestricted LPN/LVN or RN license as well as 2+ years of nursing experience.
  • Compact Nursing License required.
  • Strong clinical assessment skills with chronic disease populations (HTN, DM, CHF, COPD).
  • Excellent phone communication. You build rapport fast and explain complex things simply.
  • Comfort with technology: EHRs, RPM platforms, video conferencing.
  • Self-directed. You manage a panel without hand-holding.
  • Spanish language skills a plus.
