Job description

Physical Therapist (PT) – PRN

Location: Colonial Nursing and Rehab – Crown Point, IN

Colonial Nursing and Rehab is seeking a skilled and compassionate Physical Therapist (PT) to provide rehabilitation services at our skilled nursing and rehabilitation facility in Crown Point, IN.

The Physical Therapist implements individualized physical therapy programs and provides treatment to residents to promote increased independence, mobility, and overall functioning.

Shift Available

  • PRN

Responsibilities

  • Implement individualized physical therapy treatment plans.

  • Evaluate residents to obtain the information necessary for treatment planning.

  • Develop treatment plans that include target areas, short-term goals, and long-term goals.

  • Work with other members of the rehabilitation team to develop programs consistent with the needs and capabilities of each resident.

  • Provide physical therapy treatments designed to improve strength, mobility, balance, and functional independence.

  • Instruct residents, families, and caregivers in appropriate skills and techniques related to the physical therapy treatment program.

  • Document evaluations, treatment plans, progress, and outcomes accurately and timely.

  • Collaborate with nursing and other interdisciplinary team members to support resident-centered care.

Qualifications

  • Associate's degree in Physical Therapy from an accredited program or current Physical Therapist licensure.

  • Current Indiana Physical Therapist license in good standing.

  • Strong communication, organizational, and interpersonal skills.

  • Commitment to providing compassionate, resident-centered care.

  • Skilled nursing or long-term care experience preferred.
