Job description

Vivo HealthStaff is conducting a search for a Clinic Registered Nurse to our client's primary care clinic. As a Clinic RN, you will be responsible for coordinating and providing direct and indirect nursing care to patients at the center as well as in their homes. Working closely with the Physician, clinic staff, and other interdisciplinary team (IDT) members, you will play a vital role in managing ongoing care, complex clinical situations, and supporting the operation of the primary care clinic. Collaboration with colleagues and stakeholders to promote our values, team culture, and mission is also a key aspect of this role.

Essential Functions:

- Conduct nursing assessments in the clinic to determine the nursing care needs, preferences, and goals of the patients. Actively participate in IDT meetings to develop patient care plans.
- Visit patients' homes to deliver nursing interventions, as it is an integral part of the job description.
- Deliver and document nursing interventions based on the agreed-upon patient care plans. This includes maintaining a healthy and safe environment, promptly and accurately responding to physician orders, and correctly administering medications and performing ordered tests and treatments.
- Stay alert to input from other team members, patients, and caregivers, and promptly update the IDT of any changes in patients' condition or medical status.
- Collaborate with the Physician and other IDT members to ensure smooth care transitions between different settings (hospitals, skilled nursing facilities, home, etc.).
- Educate patients, families, and caregivers on the disease process, self-care techniques, and prevention strategies.

- Support the Marketing Team in enrolling prospective patients into the program.
- Participate in end-of-life care coordination and support.
- Provide backup coverage for other nursing functions within the clinic.
- Complete all required documentation in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information regarding employees, patients, and families.
- Adhere to company policies and procedures, and actively participate in any required Quality Improvement activities, staff training, and meetings.
- Perform other duties as assigned.

Education and Certification:

- Graduate of an accredited school of nursing; Bachelor of Science in Nursing (BSN) preferred.
- Valid California Registered Nurse (RN) License.
- Current Basic Life Support (BLS) / First Aid Certification required.

Skills & Experience:

- Minimum of two years of nursing experience in a clinical setting with a frail or elderly population.
- Nursing knowledge and skills necessary to treat frail, elderly patients and manage complex clinical situations.
- Highly motivated, self-directed, able to execute tasks in a rapidly changing environment, and make sound decisions in emergency situations.
- Excellent clinical, organizational, and communication skills when working with seniors, their families, and interdisciplinary team members.
- Experience and competency in working with people from diverse backgrounds.
- Commitment to improving the quality of life for vulnerable seniors.
- Bilingual Spanish-English preferred.
