OPEN POSITION:

Physician - Geriatric Primary Care



SCHEDULE:

- Full-Time

- Monday to Friday

- Some Flexibility Offered

- No Evenings

COMPENSATION:

- $200,000 to $250,000 Base Salary, negotiable dependent upon experience

COMPANY PROFILE:

This organization is a leading provider of integrated medical services, caring for individuals from post-hospitalization to home in various settings. Their providers make a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve.



POSITION DESCRIPTION:

The new physician will see patients in skilled nursing facilities and other long-term care settings. Providers work autonomously in the field and are remotely supported by a robust clinical and administrative team.


A low volume of patients are seen per day, allowing ample time to focus on all needs and to provide proper attention and support.


The duties of the role may include:


- provide high-quality complex geriatric care

- ensure engagement with facility leadership, other providers, patients, and families

- supervise nurse practitioners and/or physician assistants

- audit documentation for each supervised provider on a monthly basis

- collaborate with other healthcare providers as needed

- participate in the on-call schedule

- attend trainings and meetings as required


The new physician will also focus on quality outcomes including, but not limited to:


- care path compliance

- prevention of avoidable transfers

- decrease in polypharmacy

- costs of care


The practice uses an EHR on a mobile-based platform.




REQUIREMENTS:

- Indiana medical license

- DEA

- Board Certification, relevant field (Internal / Family Medicine, Geriatrics, etc.)

- experience supervising APPs

- 3+ years relevant clinical practice experience
