Social Services Assistant

We're looking for a compassionate, organized, and detail-oriented Social Services Assistant to join our team. In this role, you'll work alongside our Director of Social Services and interdisciplinary care team to enhance the quality of life for our residents while providing support, guidance, and resources to residents and their families.

What You'll Do

As a Social Services Assistant, you will:

• Welcome new residents and families by assisting with admissions and completing required paperwork.
• Conduct resident interviews and gather social history information.
• Help develop, update, and maintain individualized care plans.
• Complete documentation, assessments, progress notes, and required records accurately and on time.
• Support residents in exercising their rights, making informed choices, and maintaining dignity and independence.
• Coordinate discharge planning and connect residents with community resources and support services.
• Assist with advance directives and ensure documentation remains current.
• Monitor resident behavioral needs and collaborate with the care team on appropriate interventions.
• Participate in resident care conferences and interdisciplinary team meetings.
• Help maintain current social service resources and referral information.
• Assist the Admissions team with waitlist management, applications, assessments, and follow-up activities.
• Provide coverage for Admissions and the Director of Social Services when needed.
• Ensure PASRR screenings, documentation, and required tracking are completed accurately.

What We're Looking For

Required Qualifications

• High school diploma or equivalent.
• Ability to complete the Social Services Designee Course within six months of hire.
• Strong communication, organization, and customer service skills.
• Ability to maintain confidentiality and handle sensitive information with professionalism.
• Basic computer proficiency and experience using Microsoft Office or similar software.
• Ability to work effectively with residents, families, healthcare professionals, and community agencies.

Preferred Qualifications

• Previous clerical, administrative, healthcare, or long-term care experience.
• Experience working with older adults or in a healthcare setting.

Physical Requirements

This position requires frequent sitting, walking, computer work, communication, and documentation. Candidates should be able to lift up to 25 pounds occasionally and 10 pounds frequently, with or without reasonable accommodation.
