Job description

Access Therapy is in need of driven and dedicated Registered Nurses (RNs) for Home Health to provide excellent home care solutions to individuals of adult ages.

The Registered Nurse will use the nursing process to assess and manage the medical needs of a patient. They identify other disciplines needed to maximize patients’ progress toward goals. The Registered Nurse will be the patient’s advocate and will be in communication with the primary MD and Case Manager as the patient’s condition requires.

*We are open to hiring W2 employees, 1099 contractors, per diem, part time, full time*

QUALIFICATIONS:

  • Current active Registered Nurse license.
  • Minimum of 1 year experience preferably in a hospital or clinic setting.
  • Bilingual is a plus! [ Spanish/ Vietnamese/Chinese Speaking ]
  • Registered Nurse (1-2 years required)

Responsibilities:

  • Must be able to maintain safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel as needed.
  • Must adhere to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
  • Documents patient care services by charting in patient and department records.
  • Maintains continuity among team by documenting and communicating actions, irregularities, and continuing needs.
  • Be able to patient confidence and protects operations by complying fully with HIPAA regulations.
  • Maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
  • Must ensure that services are provided in a manner that is appropriate to patient needs and in full compliance with established medical protocols and legal requirements.
  • Provide direct patient care, including the administration of medications and immunizations, performing nursing procedures, and providing patient education
  • Be able to provide triage services, including patient assessment and education, arranging for necessary appointments and referrals, and providing appropriate follow-up. Fill-in for triage nurse as needed.
  • Be able to provide backup assistance for the Clinical Manager as needed.
  • Must be able to perform other duties as assigned

Required Skills:

  • Demonstrate clinical assessment skills at the level necessary to meet the job requirements
  • Ability to meet requirements of CPR, First Aid and other mandated programs.
  • Ability to make independent decisions when circumstances warrant such action.
  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to medical care in home health setting.
  • Proven decision-making and analytical skills.
  • Ability to organize, prioritize and efficiently carry out duties and assignments.
  • Effective interpersonal skills and the ability to work with an interdisciplinary care team.
  • Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, and policies and procedures that are necessary for providing quality care.
  • Ability to interact effectively and in a professional, supportive manner with persons of all backgrounds.
  • Excellent verbal and written communication skills.

Please note, the salary listed is for full-time employees. We do have part-time openings as well, with the pay being based off an hourly rate.
