Job description

We currently have an opportunity for a Bi-lingual Chinese-speaking Registered Nurse in Brooklyn to make visits to patients' homes.
Key Functions:
  • Perform initial assessment of clients for admission to home care, reassessments, and post-hospital discharge visits
  • Supervise and instruct PCA/HHAs
  • Review medication
  • Develop aides care plan, and MD orders in HHA Exchange
  • Collaborate with medical providers, and other members of the interdisciplinary team
  • Work closely with coordination department
  • Performs patient/family education
  • Discharge planning

Qualifications:

  • Current NYS RN License & Registration required
  • Bilingual English-Chinese required
  • Home care/experience with adult/geriatric population preferred
  • Must be willing to travel to clients homes
  • Ability to use computerized systems
  • Excellent verbal and written communication skills
  • Customer service skills are essential
