Job description


CHC is seeking a Family Medicine Physician with O.B to join the team! Responsible for providing direct outpatient, medical services to CHC patients. Renders services that reflect current standards of medical care appropriate to local medical community. Responsible for coordination of, documentation of and continuity of care and for risk management. Participates in quality assurance and quality improvement efforts.
Physicians are the clinical leaders of the care team that are responsible for providing direct outpatient medical services, including prenatal and obstetrical care, to CHC patients in support of the organizational mission. The role renders services that reflect current standards of medical care appropriate to the local medical community. Physicians are responsible for coordination and documentation of continuity of care and for risk management for patients. The physician role participates in quality assurance and quality improvement efforts, participates in lifelong learning, and supports the financial stability of the organization to continue the provision of high quality and accessible care at CHC.

Basic Qualifications

Education

  • Graduation from an accredited medical school and completion of residency training program.
  • Adequate training/experience doing prenatal care and deliveries.

Knowledge, Skills and Ability

  • Ability to work independently and solve problems creatively with initiative
  • Affinity for working with a culturally and economically diverse patient population
  • Good organizational and staff coordination skills
  • Willingness to work flexible hours

Licensure, Certification, and Other Requirements

  • License to practice medicine in State of Vermont
  • Board certification or clear path to board certification in family practice
  • Active DEA license
  • CPR certification
  • Completion of requirements for successful credentialing and privileging by CHC and applicable insurance companies, and possession of admitting and delivery priviliges at UVMMC.
  • ALSO certification in Advanced Life Support and Obstetrics

DIRECT PATIENT CARE & SERVICES

  • Provide direct outpatient, inpatient and nursing home medical services in areas of family medicine, general medicine, mental health, gynecology, family planning, pediatrics and geriatric medicine.
  • Provide prenatal care and vaginal deliveries under priviliging guidelines established by UVMMC.
  • Ensure medical, laboratory, and diagnostic follow-up on patients, including addressing communication within the EHR, and documentation of communication of results within a timeframe that is both clinically appropriate and in accordance with CHC policies. This includes checking tasks and results daily, even when not scheduled in clinic, or coordinating coverage with a colleague to ensure timely access to care and prevent delays in care.
  • Ensure access to care for patients by working to meet productivity expectations, seeing patients seeking care in available schedule time, and increasing panel size by providing access to new patients when access is available.
  • Communicate effectively and in a timely manner with patients regarding health/illness; empower patients to take active role in health maintenance; promote wellness behaviors in patients
  • Complete all clinical documentation as soon as possible and in alignment with CHC policies.
  • Refer patients to clinically necessary specialty care; ensure appropriate communication and follow-up with specialty care providers.
  • Participate in Obstetrics on-call coverage system; cover clinical responsibilities for colleagues not on duty.
  • Participate in Newborn rounding.
  • Follow CHC clinical policies and procedures

CLINICAL SUPERVISION

  • Provide clinical supervision of advanced practice providers through as-needed case consultation and scheduled chart review, including seeing patients in consultation.
  • Provide clinical supervision of nursing staff as needed for triage, medication refills, referrals and nursing services visits
  • Provide teaching to trainees, such as medical, nursing, nurse practitioner, and physician assistant students and resident physicians, as assigned

. QUALITY ASSURANCE / QUALITY IMPROVEMENT

  • Participate in quality assurance activities such as development of flow sheets and data collection instruments, chart audits and OB panel review and case conferences
  • Participate in interdepartmental quality improvement team efforts
  • Participate in efforts to improve quality outcomes defined by non-CHC entities, including UDS, PCMH, and Accountable Care Organizations
  • Contribute to CHC efforts in quality improvement efforts including higher-quality and more cost-effective care for patients, and for improved quality of work-life for staff members

CONTINUING EDUCATION

  • Participate in continuing medical education that supports CHC practice, and in quantity sufficient to meet requirements for continued licensure and board certification; submits documentation of such training to supervisor and for credentialing files.
  • Participate in developing and presenting continuing education sessions for CHC staff.
  • Attends staff and team meetings as assigned/applicable.


Physical Demands and Working Conditions

  • Work performed in an environmentally controlled office/health clinic setting
  • Ability to take over-night call in addition to daily clinical responsibilities.
  • Ability to lift/pull up to 35 lbs.
