What is a chart review nurse practitioner?

A Chart Review Nurse Practitioner (NP) is responsible for reviewing patient medical records to ensure accuracy, completeness, and compliance with regulatory guidelines. They assess documentation for medical necessity, appropriate coding, and adherence to healthcare standards. This role often supports insurance companies, healthcare organizations, or legal entities by providing expert analysis of patient charts. Unlike bedside NPs, Chart Review NPs work in a non-clinical setting, often remotely. Their work helps improve patient care quality, prevent fraud, and support reimbursement processes.

What skills and qualifications are needed to be a chart review nurse practitioner?

A Chart Review Nurse Practitioner requires a nurse practitioner license, strong clinical knowledge, and exceptional attention to detail to evaluate and interpret patient records. Experience with electronic health records (EHRs), chart auditing tools, and familiarity with healthcare coding systems such as ICD-10 and CPT are highly valuable. Strong analytical thinking, written communication, and the ability to work independently set top performers apart. These skills ensure precise, compliant chart reviews that help improve patient care, support billing accuracy, and maintain regulatory standards.

What does a chart review nurse practitioner do?

A typical day for a Chart Review Nurse Practitioner involves thoroughly reviewing patient charts to verify documentation accuracy, code diagnoses and procedures, and identify any gaps in care or compliance with regulations. Much of the work is independent, often performed remotely, though there may be regular communication with healthcare providers, coders, or quality improvement teams. You may participate in case reviews, contribute to audits, and assist providers with best practices for documentation. The routine generally emphasizes detail-oriented record analysis, collaboration to enhance care quality, and a consistent workflow that supports organizational goals.
