From advanced wound care and chronic condition management to transitional care, our experienced team brings compassionate, specialized treatment directly to patients—where they live, recover, and thrive.
We are seeking motivated Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our expanding mobile care team as independent contractors (1099). In this role, you will provide in-home and in-clinic visits, perform health assessments, manage chronic and acute conditions, and deliver advanced wound care. This is an opportunity to practice meaningful, relationship-based medicine while working autonomously in a flexible, supportive environment.
Core Responsibilities
  • Provide advanced wound care treatment for patients with complex or non-healing wounds, including thorough assessment, debridement, and individualized treatment plans.
  • Deliver mobile wound care services directly in patients’ homes or care facilities, ensuring comfort, accessibility, and continuity of care.
  • Diagnose and manage chronic and acute conditions using a comprehensive, whole-patient approach.
  • Accurately document all patient encounters, treatments, and progress in the EMR system in accordance with organizational standards.
  • Actively participate in care planning and coordinate with internal and external support teams to optimize outcomes.
  • Ensure strict compliance with Medicare, state regulations, and organizational protocols governing wound care services.
  • Other duties as assigned.
Qualifications
  • Active and unrestricted Nurse Practitioner or Physician Assistant license in Nebraska or Iowa.
  • Experience in wound care, home-based care, or geriatric medicine preferred.
  • Reliable transportation and valid driver’s license. The position involves day-trip travel to patient homes and partner facilities.
  • Strong clinical assessment, communication, and documentation skills.
  • Self-starter comfortable working independently with mobile technology.
