Jackson Therapy Partners is seeking a travel Outpatient Physical Therapist for a travel job in South Bend, Indiana.

Physical Therapist – Outpatient (Travel)

Join a South Bend, IN outpatient clinic as a traveling Physical Therapist. Evaluate patients, develop and modify treatment plans according to patient needs and physician direction, and provide discipline-specific supervision and delegation. Deliver functional treatment programs that support return to community activities. Serve as an educator to patients, families, students, and coworkers. Maintain professional standards, confidentiality, and compliance with organizational policies and applicable codes of conduct.

Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Indiana physical therapy license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience in outpatient physical therapy settings preferred.
  • Demonstrated competency in comprehensive patient evaluation, treatment planning, documentation, and use of functional outcome measures.
  • Ability to complete timely, accurate medical record documentation, letters of medical necessity, and charge entry in compliance with regulatory and clinic requirements.
  • Willingness to participate in student clinical education and provide appropriate supervision and evaluations (preferred).
  • Maintain required continuing education and adherence to state Practice Act and professional code of conduct.

Preferred Qualifications

  • Previous travel therapy experience.
  • Experience with safe patient handling practices and materials handling protocols.
  • Familiarity with performance improvement initiatives and participation in quality improvement activities.
