Job description

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

Outreach 

  • Outreach to uninsured patients at CFH by phone, mail, or in-person to promote health insurance enrollment. 

  • Document outreach encounters in eClinical Works using telephone encounters. 

  • Provide structured patient education on Medicaid and other insurance eligibility. 

  • Develop and distribute appropriate outreach materials to patients and partner organizations to increase insurance coverage option awareness and applicable Affordable Care Act (ACA) provisions. 

  • Travel within CFH health centers to provide insurance enrollment, as required. 

Health Insurance & Housing Enrollment 

  • Provide insurance enrollment assistance for uninsured and underinsured patients, including but not limited to completing applications, gathering required documentation, and troubleshooting the enrollment process. 

  • Utilize health insurance verification systems such as ePaces, Claim Remedi and EBO reports to verify health insurance information. 

  • Work with NY State of health Marketplace to submit applications, client’s documents, and file complaints, as necessary. 

  • Document insurance application encounters in eClinical Works. 

  • Collaborate with partner organizations to increase enrollment in Medicaid, Medicare, and other health insurance. 

  • Attend and successfully complete all required training programs and participate in on-going training and webinars, as necessary. 

  • Assist in the development and implementation of organizational outreach and enrollment initiatives. 

  • Perform additional duties, as required. 

Mental Health Services 

  • Educate patients in mental health services available within CFH, and beyond. 

  • Refer patients in need of mental health services to a mental health provider at CFH and in the community. 

  • Search for organizations and programs based on client’s need.  

Administrative Responsibilities 

  • Conduct training for new and current staff on health insurance verification and documentation. Training will take place in person and/or virtually, as needed. 

  • Collaborate in development of training materials for new and current staff on insurance documentation such as ePaces and Claim Remedi user guide and examples.  

  • Assist in the creation of promotional materials to distribute and post at CFH health centers to promote insurance enrollment among CFH clients.  

  • Assist with the onboarding and training of new Eligibility and Enrollment Specialists which include but are not limited to reviewing applications process, eCW documentation and ePaces/Claim Remedi platforms.  

  • Assists in the ordering and distribution of incentives for the EE team.  

  • Carry out other duties as assigned by Senior Care Coordinator and/or Program Manager.

Education:  

  • Bachelor's degree in Social Work, Public Health, or a related field preferred.  

Experience:

  • Minimum of 2 years of experience in housing services, case management, or a similar role. 

  • Previous experience providing services and exercising leadership in a culturally and linguistically diverse setting, with demonstrated success. 

  • Prior experience in a health care setting, preferably FQHCs, strongly preferred.  

  • Knowledge of HUD regulations and experience with HUD-funded programs (preferred). 

  • Proficiency in using Electronic Health Records (EHR) systems or housing management software. 

  • Excellent organizational and communication skills. 

  • Ability to work effectively with a diverse population. 

  • Strong problem-solving skills and ability to respond to crises. 

  • Languages:  Bilingual English/Spanish/French strongly preferred.
