Shift:

  • Nights: 10:30 PM โ€“ 7:00 AM

Staffing

Charge Nurse
  • Charge nurse assigned depending on census.
  • One float RN may assist with:
    • Admissions
    • Discharges
    • IVs
    • General support
Nurse Aides
  • Days/Evenings: 2โ€“3 CNAs
  • Nights: 1โ€“2 CNAs
  • Staffing based on census
RN/LPN Staffing
  • Example:
    • Census of 18 = 2 nurses with 9 patients each.
    • Higher census (20โ€“32 patients):
      • 3 nurses on days/evenings
      • 2 nurses on nights
  • Charge nurse is responsible for all patients on the unit.
  • Since COVID, census has generally been capped around 21 patients, so patients are not currently cohorted.

Unit Overview

This is a post-acute Transitional Care Unit licensed under Long-Term Care (LTC) regulations. It serves as a step-down unit for patients who are medically stable but not yet ready for discharge home or to a skilled nursing facility.

Average Length of Stay:

  • Approximately 14 days

Patient Population

  • Primarily orthopedic patients (especially total knee and hip replacements)
  • Other non-acute, non-critical medical and surgical patients
  • All patients must have a skilled nursing need

Common Nursing Responsibilities

  • Post-operative care
  • Respiratory care and interventions
  • Wound care and dressing changes
  • IV antibiotic administration
  • Coordination of scheduled therapy sessions
  • Admissions and discharges
  • General skilled nursing care in a post-acute setting
