About the Role
Clarvida is seeking a CBRS/Case Manager to provide skill-building and behavioral health support to clients in their homes and communities. You will serve clients ages 5 through adult who may experience behavioral challenges, emotional disturbance, or severe and persistent mental health diagnoses.
In this role, you will meet with clients weekly to focus on developing coping strategies, building independence, improving life skills, and accessing vital community resources. Documentation-including daily progress notes, treatment plan updates, and discharge summaries-is an essential part of the position. Thisflexible-schedule role offers meaningful one-on-one work and strong opportunities for growth.
Responsibilities
Provide weekly community-based and in-home skill-building sessions
Support clients with behavioral health needs, emotional disturbance, and chronic mental illness
Help clients develop coping skills, independence, and long-term stability
Connect clients with community resources and supports
Maintain accurate documentation including progress notes, treatment plans, and discharge summaries
Collaborate with supervisors, clinicians, and community partners as needed
Required Qualifications
Bachelor's Degree (Human Services field preferred: Social Work, Psychology, Sociology, Counseling, Family Relations, Nursing, OT, PT, TR, SLP, Behavioral Sciences, Art Therapy, etc.)
Ability to pass background checks, including Enhanced Fingerprinting with Idaho DHW
Valid driver's license, reliable transportation, and proof of auto insurance
Experience with electronic health records (EHR) is a plus
