A Brief Overview
The Clinical Pharmacist at SVH provides comprehensive pharmaceutical care to patients birth to death by optimizing medication therapy, providing disease state prevention and management and promoting health and wellness. This mission is accomplished by upholding the American College of Clinical Pharmacist philosophy of pharmaceutical care; blending a caring orientation with specialized therapeutic knowledge, experience and judgment for the purpose of ensuring optimal patient outcomes. The Clinical Pharmacist also contributes to the generation of new knowledge that advances health and quality of life.
Education Qualifications
  • Bachelor's Degree Bachelor of Science in Pharmacy from an accredited school of pharmacy. Required or
  • Doctorate PharmD from an accredited school of pharmacy. Required

Experience Qualifications
  • Successful completion of an ASHP accredited residency plus one year of hospital or relevant clinical experience Required or
  • completed equivalent clinical followship Required or
  • 3 years Relative work experience that includes essential duties at healthcare system Required

Skills and Abilities
  • Medical terminology and knowledge of medication names (Required proficiency)
  • Ability to apply mathematical concepts and apply mathematical operations to such tasks as analysis of variance, correlation techniques, sampling theory, and factor analysis. Also, use of spreadsheets, internet/e-mail, database software, contact management, and the electronic medical record (Required proficiency)
  • Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations. (Required proficiency)

Licenses and Certifications
  • Pharmacist - KBP Must be licensed within 90 days of hire. Required
  • Basic Life Support - BLS Required within 90 days of hire.
  • Advanced Cardiac Life Support - ACLS Required within 90 days of hire.
  • Vaccine certification required within one year of hire Required
  • ASHP certification within 2 years of hire is required or clinical certification from an accredited organization or board certificate within 2 years of hire Required

What you will do
  • Review, interpret and verify provider medication orders making any recommendations and clinical interventions as necessary based on patient specific conditions, disease states, age and other clinical factors such as safety and cost effectiveness. Complete assigned pharmacy consults proficiently and following all pharmacy policies and procedures.
  • Assess the status of the patient's health problems as possible and determine whether the prescribed medications are optimally meeting the patient's needs and goals of care. Recognize untreated health problems that could be improved or resolved with appropriate medication therapy. Follow the patient's progress to determine the effects of the patient's medications.
  • Lead, co-lead or participate in hospital/pharmacy committees upon assignment by clinical coordinator/ manager or pharmacy director according to the specialty area covered such as antimicrobial stewardship, opioid stewardship, anticoagulation committee...
  • Precept, educate and train pharmacy personnel/students/residents and other healthcare professionals, and exhibit qualities of servant leadership.
  • Participate in at least 50% of assigned schedule engaged in patient care planning within a multidisciplinary team in respective areas. Respond to emergent calls such as code blue and stat calls. Provide a consistent process of patient care that ensures the appropriateness, effectiveness, and safety of the patient's medication use.
  • Support the health care team's efforts to educate the patient on other important steps to improve or maintain health, such as exercise, diet, and preventive steps like immunization. Advise the patient on how to best take his or her medications.
  • Develop and conduct medication use evaluations as needed, and report results to the relevant committees or subcommittees to implement in daily practice.
  • Design, implement, and lead pharmacy process improvement initiatives, including those focused on evidenced based care, safety, or financial sustainability.
  • Participate in an original research study and submit for publication (e.g., journal articles, posters, presentations)
  • Active participation in a clinical organization of pharmacy (e.g., ACCP, KCHP, ASHP)
