RN Case Manager-{169878} Job Title: RN: Case Manager Location: Jeffersonville, IN 47130 · Work Area/Coverage: Harrison, Clark, Crawford, Orange, partial Floyd & Washington counties (plus additional areas as needed for PTO/call-ins/admissions) Pay: $56.14/hour · Mileage Reimbursement: $0.46/mile Schedule: Monday-Friday, 8:00am-5:00pm + weekend on-call rotation Overview We’re hiring an RN Case Manager to complete comprehensive assessments for high-risk members and coordinate care across providers and settings. This role focuses on care planning, care transitions, patient advocacy, and driving quality outcomes through effective clinical case management.
