A case manager provides aid and guidance for clients who need social services or other assistance. As a case manager, you assess the needs of each client through one-on-one conversation, documents, and research and then connect them with the appropriate agency, person, or service best equipped to help. You can coordinate medical or mental health care services, recommend economic assistance programs, or help with issues such as housing, childcare, or personal development. Your duties and responsibilities also include meeting with clients to track their progress and adjust goals and plans if necessary.
