SUMMARY

The SOAR Case Manager will provide case management/therapeutic services to consumers with severe and persistent mental illness (SPMI). In coordination with therapeutic staff, the incumbent will assist consumers by providing information, education, and training to build knowledge, develop skills, and change attitudes with a goal of increasing the consumerโ€™s independence, productivity, self-determination, integration, and inclusion in the community. The majority of the Case Managerโ€™s time will be spent providing services to consumers in consumerโ€™s homes, vocational site, program site or other community settings, with minimal time spent at the Center. This position reports directly to the CSS Director or designee.


MAJOR JOB RESPONSIBILITIES:

  • Provide SOAR case management for clients seeking Social Security Benefits
  • Provide SOAR training when necessary
  • Provides case management to SPMI adults supporting consumerโ€™s treatment plan goals and advocacy in the areas of: Education/Vocation, Residential, Financial, Health, Leisure, and Social Supports utilizing community resources on behalf of the consumer as needed
  • Collaborates at the local and state level on behalf of ELC in coordinating services and resources for SPMI consumers
  • Assists in collection of program data and provides reports as necessary to immediate supervisor
  • Works in conjunction with other mental health center staff
  • Participates in treatment planning based on best practice theories and applies interventions
  • Meets direct service expectations
  • Meets deadlines for daily paperwork and reports
  • Provides quality documentation
  • Provides supportive intervention when needs arise outside of scheduled hours
  • Completes other duties as directed
  • Completes all required trainings as assigned
  • Complies with all ELC policies and procedures

Requirements:

EDUCATION and/or EXPERIENCE:

  • Bachelorโ€™s degree or equivalent experience necessary to meet State of Kansas licensing regulations
  • Possesses the minimum qualifications as documented in the Kansas Case Management Program Standards
  • Familiarity with mental health theory and practice
  • Medicaid experience a plus
  • Possesses valid driver's license and good driving record
  • Ability to pass all required background checks
