Job description

Job Title: SOAR Case Manager

Employment Type: Full-time

FLSA: Non-Exempt (Hourly)

Pay Range: $22.00 - $26.00 per hour (based on experience)

Position Summary:

The SOAR (SSI/SSDI Outreach, Access, and Recovery) Case Manager at Vivia Health supports adults experiencing or at risk of homelessness who have serious mental illness or co-occurring disorders. This role is focused on assisting clients in accessing federal disability benefits using the SOAR model. The case manager works closely with internal clinical teams and external agencies to ensure effective, compassionate, and timely application support.

Key Responsibilities:
  • Screen clients for SOAR eligibility based on homelessness status, mental health conditions, and income.
  • Gather necessary documentation, including medical, psychiatric, and identification records.
  • Complete all relevant SSA forms (e.g., SSA8000, SSA827, SSA1696) accurately and promptly.
  • Draft a Medical Summary Report (MSR) detailing functional impairments and diagnoses, using clinical language and referencing DSM5 criteria.
  • Collaborate with healthcare providers to obtain cosigned medical statements supporting the clientโ€™s application.
  • Submit a complete SOAR application packet to SSA and DDS, and maintain followup communication.
  • Track applications, respond to SSA/DDS inquiries, and advocate for resolution in case of delays.
  • Provide postdecision support, including appeals and connection to housing or other benefit
  • Document all activities, contacts, and outcomes in Athena EHR with accuracy and timeliness.
  • Participate in ongoing SOAR trainings, maintain certification, and attend supervision meetings.
  • Qualifications:
  • Bachelor's degree in social work, Psychology, or related field (Masterโ€™s preferred).
  • Familiarity with SSI/SSDI application processes and mental health case management.
  • Prior SOAR training or certification is highly preferred.
  • Strong written communication, organizational, and interpersonal skills.
  • Experience working with vulnerable or homeless populations in communitybased settings.
  • Experience:
  • 2+ years of experience in case management, crisis intervention, healthcare, and/or social services.
  • Experience with IMCANS assessments and acute mental illness a plus.
  • Skills & Abilities:
  • Strong organizational skills and attention to detail.
  • Excellent communication and interpersonal skills, with the ability to build rapport and advocate for clients effectively.
  • Ability to handle sensitive and confidential information with professionalism and discretion.
  • Skilled ability to track patients and needs to support continuity of care and ensure follow up with needs/services.
  • Strong electronic documentation skills required (EHRs, Microsoft Teams, Excel, and SharePoint).
  • Ability to work independently and manage multiple tasks in a dynamic environment.
  • Bilingual Spanish with proficiency in translating medical terminology highly desirable.
