Job Description
We are seeking a dedicated and experienced Case Management Registered Nurse to join our team. In this role, you will collaborate with a multidisciplinary team to manage patient care effectively, ensuring the highest quality and efficiency throughout their journey.
Responsibilities
  • Coordinate patient care and collaborate with healthcare professionals.
  • Assess and manage care plans for patients.
  • Facilitate communication among patients, families, and healthcare teams.
  • Ensure utilization of resources and compliance with standards.
Required Experience / Certifications / Licensure
  • Must hold a valid Registered Nurse license.
  • Experience in case management is required.
  • Familiarity with Meditech and Midas preferred.
