Job description

We are looking for a Part Time Case Management Assistant to join our team!

What you will do:

  • The Case Management Assistant will work collaboratively with patients and Case Managers to communicate and document accurate clinical and regulatory information.
  • Collaborate with internal and external partners to address financial advocacy for the patient/organization.
  • Audit Charts for Medicare requirements and communicate regulations effectively
  • Assist with follow-up appointments and telephone calls and other post acute/transitional planning care needs.
  • Support coordination with representatives from insurance companies, accountable care organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies.

What you will need:

  • High School Diploma or GED requires. Associate's Degree preferred.
  • Two years to five years prior experience a health related field.
  • Associates degree in Human Services Field preferred.
  • Insurance/payer experience preferred.
  • Basic knowledge of Medical Terminology.

What you need to know:

  • Part Time Opportunity 16 hours
  • Must be available to work between 7:30 AM- 4:00 PM, Monday through Friday, and every other weekend , including one holiday a year.
  • Location - Baystate Medical Center- 759 Chestnut Street, Springfield.

Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.

$20.79 - $23.90 - $28.08
