Job description

Aloma Healthcare, Inc. seeks a talented and experienced Care Managers to join us for a full-time role as an individual contributor. The Care Manager will be responsible for leading patient care teams and ensuring provision of quality healthcare services to all patients. The Care Manager must possess excellent communication, problem-solving, and customer service skills, as well as the ability to remain organized and multitask.
Responsibilities
• Lead patient care teams to ensure provision of quality healthcare services
• Develop team performance plans, goals, and objectives
• Oversee patient care services, including scheduling, tracking, and evaluating
• Monitor patient care processes and adjust as needed
• Maintain up-to-date records of patient care services
• Ensure compliance with all healthcare laws, regulations, and policies
• Monitor patient care teams to ensure quality and compliance with safety protocols
• Communicate proactively with patients, management, staff, and other healthcare professionals to foster positive relations
• Generate KPIs (key performance indicators) and PIP (performance improvement projects) and metrics to improve company base and client level improvements
• Conduct and oversee quality meetings (QAPI) and conduct compliance operation audits (client and staff files)
• Attend conferences and marketing events and manage media relations/external communications
• 50% of assignments will involve home care responsibilities
Requirements
• Bachelor's degree in related healthcare field
• 5+ years of professional healthcare experience
• Strong knowledge of healthcare regulations and laws
• Excellent communication, problem-solving, and customer service skills
• Proven ability to lead teams in a positive, productive manner
