The Care Manager will be responsible for leading patient care teams and ensuring provision of quality healthcare services to all patients. The Care Manager must possess excellent communication, problem-solving, and customer service skills, as well as the ability to remain organized and multitask.
Responsibilities
• Lead patient care teams to ensure provision of quality healthcare services
• Develop team performance plans, goals, and objectives
• Oversee patient care services, including scheduling, tracking, and evaluating
• Monitor patient care processes and adjust as needed
