Cardinal Senior Care is expanding in The Woodlands market, and we are looking for a proven outside sales professional who knows how to build referral relationships, generate private-pay home care referrals, and grow weekly care hours.
This is a field-based sales role for someone who enjoys being out in the community, walking into referral accounts, building trust, following up consistently, and turning relationships into real client starts.
We are especially interested in candidates with experience in private-duty home care, home health, hospice, senior living, rehab, skilled nursing, hospital discharge planning relationships, or healthcare referral-source sales.
About Cardinal Senior Care

We are redefining senior wellness through our exclusive Trilogy Program and AI-powered care technology. We partner with hospitals, rehabs, hospices, senior living communities, physicians, and other healthcare professionals to support seniors and families during important care transitions.
The Role
As the Outside Sales Representative for The Woodlands area, you will be responsible for growing Cardinal Senior Care’s referral network and generating new private-pay home care clients.
You will own the assigned territory and be expected to build strong relationships with professional referral sources, identify new opportunities, follow up consistently, and work closely with operations to help convert referrals into active clients.
This role is best suited for a competitive, organized, relationship-driven salesperson who wants to build a major growth territory and be measured by results.
What You Will Do

  • Build referral relationships with hospitals, rehab centers, skilled nursing facilities, assisted living communities, memory care communities, hospices, home health agencies, physician offices, elder law attorneys, care managers, and senior-focused organizations.
  • Develop and execute a weekly territory plan for The Woodlands, Spring, Conroe, Shenandoah, Oak Ridge North, and surrounding areas.
  • Generate qualified referrals for non-medical home care services.
  • Convert referral relationships into new client opportunities and active starts of care.
  • Represent Cardinal Senior Care at networking events, community events, professional meetings, and senior care-related activities.
  • Become a trusted resource for discharge planners, social workers, case managers, senior living teams, and healthcare professionals.
  • Track all sales activity, referral sources, follow-up, pipeline, and results in the CRM.
  • Work closely with the operations team to ensure new clients are started successfully and referral partners receive timely follow-up.
  • Grow new weekly care hours and new client starts month over month.

What Success Looks Like

  • Success in this role means you are consistently in the field, developing strong referral relationships, and producing measurable growth.
  • You will be expected to:
  • Make consistent in-person referral-source visits every week.
  • Build a strong referral-source pipeline within the first 90 days.
  • Generate qualified referrals from professional referral sources.
  • Help convert private-pay families into active Cardinal Senior Care clients.
  • Grow new weekly care hours month over month.
  • Maintain accurate CRM documentation and disciplined follow-up.
  • Communicate professionally with families, referral partners, and internal team members.
  • Represent Cardinal Senior Care with urgency, integrity, and follow-through.

Required Experience
  • Minimum 2 years of outside sales, business development, healthcare sales, or referral-development experience required.
  • Strong preference will be given to candidates who have already generated referrals, admissions, client starts, or weekly care hours in one or more of the following areas: Private-duty home care, Home health, Hospice and Senior living
  •  Skilled nursing / rehab
  •  Hospital or discharge planning relationships
  •  Healthcare business development
  •  Senior care referral development
  • Candidates should be prepared to discuss actual referral sources, territory strategy, weekly activity levels, production numbers, and sales results from prior roles.

  •  Experienced in referral-based selling
  •  Skilled at building long-term professional relationships
  •  Persistent without being pushy
  •  Organized and disciplined with follow-up
  •  Comfortable being measured by activity, referrals, new clients, and weekly care hours
  •  Able to manage a territory independently
  •  Professional, warm, trustworthy, and responsive
  •  Comfortable using CRM tools
  •  Willing to be in the field daily across The Woodlands and surrounding areas
