Human Touch Home Health is seeking a Physical Therapist (PT) for PRN home health opportunities throughout Carbon County, Pennsylvania.

Patient opportunities may include Jim Thorpe, Lehighton, Palmerton, Weatherly, Nesquehoning, Summit Hill, Lansford, Albrightsville, and surrounding communities.

What You'll Do
  • Complete physical therapy evaluations in patients' homes
  • Develop individualized plans of care
  • Provide therapeutic exercise, gait, balance, transfer, and mobility training
  • Assess home safety and fall risks
  • Educate patients and caregivers
  • Monitor patient progress and adjust treatment plans
  • Complete progress assessments and discharge planning
  • Coordinate care with nurses, physicians, and other team members
  • Complete accurate and timely documentation using WellSky
Qualifications
  • Active Pennsylvania Physical Therapist License Required
  • Graduate of an accredited Physical Therapy program
  • Valid Driver's License and reliable transportation
  • Strong clinical assessment and communication skills
  • Ability to work independently in patients' homes
  • Home Health Experience Preferred, but Not Required
  • WellSky Experience is a Plus
