Job description

For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate, philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and physical care. The hospice receives patients from referring institutions throughout New York City and the entire tristate area. Primary responsibility to provide care to patients and participate in review, assessment, committee and educational functions as required.
Safety
  • Elicits recommendations regarding continuous improvement of patient safety from staff.
  • Encourages reporting of occurrences without fear of reprisal.
  • Participates in the development of recommendations for improvement in patient safety based on staff recommendations.
  • Participates in the implementation and monitoring of safety program.

Planning
  • To participate in developing policies and procedures for patient care and quality assessment and performance improvement.

Staffing/Performance
  • To perform such additional duties as may be assigned by the Medical Director and Executive Medical Director.

Quality
  • To provide primary care to those patients admitted to Calvary Hospital.
  • To perform and record a history and physical examination on all patients admitted.
  • To make daily rounds on all patients; write orders as appropriate; and document findings and plan of care/treatment in the medical record.
  • To share on-call coverage weekdays, weekends, and holidays.
  • To prepare a complete and legible medical record for each patient, whose content shall be sufficient to justify the diagnosis, warrant the treatment and end result, and include plans for post-hospital care where appropriate.
  • To abide by Calvary Hospital and Medical Staff By-laws, Rules & Regulations, Policies & Procedures, and duties as stated in hospital and department manuals.
  • To participate in departmental and Hospital quality review, assessment, and performance improvement functions.

Communication
  • To maintain ongoing communication with family members/significant others regarding patients= prognosis, the plan of care/treatment, medications, etc.
  • To participate in interdisciplinary assessment and care of all patients with Nursing, Social Work, Nutritional Services, Pastoral Care, Clinical Review/Case Management, etc., as appropriate.
  • To ensure continuity of patient care by maintaining ongoing communication with covering physicians and other members of the interdisciplinary care team.
  • To provide liaison to the Calvary Hospital Department of Medicine; to participate in Medical Staff meetings and other meetings at the discretion of the Medical Director and Executive Medical Director.
  • To cooperate fully in the overall functions and activities of Calvary Hospital.
  • To cooperate with/coordinate The Joint Commission functional activities with advice, direction, and counsel.
  • Works within legal, ethical practice standards to his/her position as well as Calvary's Code of Conduct. Complies with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer.

Requirements
Qualifications:
  • New York State M.D. License.
  • Certified by the American Board of Internal Medicine or the American Board of Family Practice, or have demonstrated substantially equivalent training and expertise.
  • Competent to care for patients suffering from advanced cancer.
  • Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.

Education:
  • Medical degree from an approved educational institution.
