At the Center for Sports Medicine & Orthopaedics (CSMO), our C4 therapy locations offer a unique blend of clinical excellence and performance-driven rehabilitation. We proudly serve patients of all ages, but we are especially known for working with athletes and active adults who want to get back to doing what they love - safely, quickly, and at the highest level possible.

We're seeking skilled, motivated PRN Physical Therapists who thrive in an energetic environment and want access to tools, space, and resources that go far beyond traditional outpatient clinics.

Your Role & Impact

As a PRN Physical Therapist with C4, you will help patients return to high-level performance using advanced rehab tools and evidence-based practice. Whether it's a competitive athlete returning to their sport, an active adult rehabbing an injury, or a motivated patient working toward long-term strength and mobility, your expertise directly shapes their outcomes.

Key Responsibilities
  • Perform comprehensive evaluations and create individualized treatment plans
  • Deliver evidence-based interventions in both clinic and performance spaces
  • Utilize turf, court, and weight room equipment for functional and sport-specific rehab
  • Collaborate with PTAs, athletic trainers, physicians, and support staff
  • Document accurately and maintain high-quality patient communication
  • Float between C4 Chattanooga and C4 Cleveland therapy locations as needed

Minimum Qualifications
  • Licensed Physical Therapist (PT)
  • BLS (CPR) certification preferred
  • Strong communication and customer service skills
  • Prefer one year of outpatient orthopaedic experience
  • A genuine passion for serving athletes, active adults, and the outpatient orthopaedic population
  • A growth‑minded clinician who values lifelong learning, professional development, and staying current with evidence‑based practice
  • Ability to travel between both C4 locations
