POSITION SUMMARY

The AIC Case Manager provides comprehensive pro-social and strengths-based case management services for adults involved in the criminal justice system. Duties include community linkages, fulfillment of basic needs, use of motivational interviewing and strategic case management approaches to reduce recidivism and support client success, andย reporting client outcomes to court/probation/parole. Significant training and quality assurance offered.

EDUCATION AND EXPERIENCE/QUALIFICATIONS

BA/BS degree required. Approved exceptions may be made for demonstrated ability and/or exceptional performance within a CSSD (Court Support Services, Judicial Branch) funded or Wheeler Clinic program, as well as substitution of experience for education on a year for year basis.

Looking for a male candidate for toxicology screenings.ย 

Displays excellent work habits (attendance, promptness, timeliness, professionalism), is open to feedback, and makes good use of time. Proven ability to work on a team to ensure program goals are met, communicates effectively verbally and in writing, and is committed to personal and program growth.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Conducts client intakes including orientation, paperwork, psychometric assessments, assessment review, service recommendations based on clientโ€™s risk, needs, strengths and responsivity factors, and client-centered Individualized Service Plans (ISPs) within established timeframes.

Provides individual case management sessions to increase the probability of service completion, goal attainment and effective life functioning. Duties include community linkages, use of standardized case management approaches (e.g., Strategic Case Management, Life Skills) and tools (e.g., Carey Guides), and vocational and educational support.

Develops Transition Plans that include connection to relevant community resources and prosocial supports designed to enhance and maintain client success.

Prepares, plans, and facilitates individual and group sessions, including booster sessions, with integrity and fidelity to curriculum in a solution-oriented manner.

Coordinates client care within agency and with other community providers.

Assesses clientsโ€™ basic needs, providing essential items and connecting clients to services and agencies that assist with basic needs. Maintains and updates community resource guide. Networks in the community, as determined by CSSD.

Utilizes motivational interviewing techniques to engage clients and their natural supports in AIC programming.

Monitors client behaviors and uses behavior management systems. Holds clients accountable and addresses ambivalence and resistance while helping clients maintain motivation to change.

Maintains accurate and timely records of case management activities through entry into agency and funder data systems.

Prepares reports that are accurate and timely, and updates referral sources on client activities on a regular basis.

Actively participates in quality assurance activities including audio taping sessions and meeting with a quality assurance coach, attending trainings, and data report analysis and review.

Participates in efforts to meet and maintain performance-based measures set by agency and CSSD.

Participates in client-centered, strengths-based, presentations/discussions in staff meetings and attends teaming meetings with Probation, as required.

Models pro-social behaviors and serves as a role model to clients and staff.

Operates as part of a team by providing program coverage for client services (including urinalysis) outside of normal work schedule, typical job function or primary work site, as requested (e.g., additional evening shifts may be required to ensure operational coverage).

Supervises toxicology testing. Supervised urine specimens shall only be collected from a client by a staff member of the same gender.

Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.

Regulatory and Corporate Compliance Responsibilities

Understands and adheres to Wheeler Clinicโ€™s compliance standards as they appear in the clinicโ€™s Corporate Compliance Plan, Code of Conduct, and Conflict of Interest Policy.

Completes all training in a timely manner as required by state and federal regulations (e.g., Compliance, Fire Safety, HIPAA HITECH, Infection Control, Sexual Harassment, etc.) and specific to the department and position.

Performs other related duties as assigned.
