Our Client has an immediate need for a Bachelor Level Case Manager in Boston, MA.
SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Provide clients with clinical services, including case management, individual treatment or group work
* Complete all documentation including assessments, Relapse Prevention Plans, and Individual Treatment Plans
* Oversee the intake process
* Determine appropriate transition and discharge plans for all residents
* Develop individualized behavior management plans and support staff to understand the individual youth treatment concerns
* Facilitate case conferences and discharge planning for all residents
* Assist with family engagement, permanency planning and life-skills development for all residents.
* Provide on-call support
CLIENT'S REQUIRED SKILLS AND EXPERIENCE
* Bachelors (Master's preferred) in social work or human services field
* At least 3 years of clinical experience
* Prior familiarity with Massachusetts child welfare systems and / or DCF
* Must have valid driver's license and vehicle
