Job description

Position Summary:
Acclaim Home Health Care is seeking a skilled and compassionate Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting. This role involves performing a wide range of clinical duties including wound care, IV therapy, comprehensive assessments, and care coordination. The RN will be responsible for completing Start of Care (SOC), Resumption of Care (ROC), and ongoing skilled nursing visits, while ensuring accurate and timely documentation in compliance with home health regulations.
The ideal candidate is organized, clinically strong, and experienced in home health, with a commitment to delivering exceptional care in a community-based setting.
Key Responsibilities:
  • Perform Start of Care (SOC), Resumption of Care (ROC), and recertification visits in accordance with physician orders
  • Conduct comprehensive patient assessments and develop individualized plans of care
  • Provide skilled nursing services including wound care, IV therapy, medication management, and disease education
  • Monitor patient conditions and communicate changes to physicians and care teams promptly
  • Educate patients and caregivers on treatment plans, medications, and safety practices
  • Complete all required documentation accurately and in a timely manner (OASIS, visit notes, care plans, etc.)
  • Ensure compliance with all state, federal, and agency regulations
  • Collaborate with interdisciplinary team members to coordinate and optimize patient care

Qualifications:
  • Active Registered Nurse (RN) license in the state of Massachusetts (required)
  • Valid driver's license and reliable transportation (required)
  • Previous home health experience (preferred)
  • Strong clinical skills including wound care and IV therapy
  • Experience with OASIS documentation and electronic medical records
  • Excellent assessment, organizational, and communication skills
  • Ability to work independently and manage a flexible schedule
